Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
Our office is happy to assist you in the filing of your insurance claims and pre-authorizations. We can even help you decide when recommended treatment can be planned over a period of time in order to help you maximize your insurance benefits as well. We require that any estimated deductibles and/or co-payments be paid in full at the time of your visits. Please be aware that our estimates are based on the most current information we have on file regarding your insurance coverage, but it is ONLY AN ESTIMATE. Final determination can only be made after your insurance carrier has processed your claims. Be aware that the patient remains financially responsible for all payment of all dental services which have been provided to them. To prevent potential misunderstandings, we ask our patients to accept and adhere to our financial policy unless prior arrangements have been made.
